Reconstruction after Excision of Hidradenitis Suppurativa: Are Skin Grafts Better than Flaps?

چکیده
1 H idradenitis suppurativa is an apocrine adenitis characterized by infection of hair follicles, which causes painful induration, sloughing, and pus drainage. Fistula formation caused by reeruptions leads to refractory and chronic disease. Although surgical treatment is considered the best option for advanced hidradenitis suppurativa,1,2 a high rate of recurrence has been reported even with wide excision and various reconstruction methods.1 Currently, there is still no consensus regarding the optimal surgical strategy. In fact, a recent systematic review and meta-analysis reported that there is insufficient evidence to show the advantages of flaps over skin grafts although they are superior to primary closure.3 In this report, we present a case of hidradenitis suppurativa in the bilateral perineal region in which reconstruction with a skin graft clearly had a better postoperative outcome than that with a skin flap. This experience provides an opportunity to reconsider the optimal reconstruction method after excision of intractable hidradenitis suppurativa in terms of recurrence.

Reconstruction after Excision of Hidradenitis Suppurativa: Are Skin Grafts Better than Flaps?
After surgical excision of hidradenitis suppurativa, reconstruction with a skin graft or a flap is performed when primary closure is not possible. However, the recurrence rate is reportedly high even after wide surgical excision. It is still unclear which reconstruction method provides the lowest recurrence rate. In this report, we present a case of intractable hidradenitis suppurativa in the b...

Hidradenitis suppurativa is a chronic inflammatory disease of the sweat glands and hair follicles which is rarely associated with squamous cell carcinoma (SCC). A case of vulval SCC complicating hidradenitis suppurativa is presented. In addition to being the first case to report the association of vulval SCC and hidradenitis suppurativa in the English language literature, it also illustrates th...

Acne inversa (AI, hidradenitis suppurativa, Velpeau's disease, Verneuil's disease) is a severe, chronic inflammatory dermatosis of unknown etiology, detected on the basis of clinical symptoms more frequently in women than in men. Purulent lesions in the form of nodules and inflammatory tumors, fistulas and scars are present in the areas with hair follicles and apocrine glands, most commonly on ...

Axillary hidradenitis suppurativa is a chronic recurrent disorder characterized by abscessing inflammation, fistulating sinus tracts, and scarring. This is not only a medical disease, but also a complicated problem necessitating surgical intervention. While various management strategies have been reported, the results are frequently unsatisfactory. Primary closure of the defect after radical ex...
متن کاملBilateral Pedicled Superficial Inferior Epigastric Artery Flap in the Treatment of Hidradenitis Suppurativa
The authors present a new technique in surgically treating hidradenitis suppurativa (HS), a debilitating skin condition. In HS, surgical treatment is often the best option because of the high recurrence rates despite extensive medical treatment. A commonly successful surgical method is using skin flaps after excision of the affected area. A superficial inferior epigastric artery flap is demonst...
